As permitted by federal law, NDT Residential is no longer automatically mailing federal Forms 1095-C. If you wish to receive a copy of your Form 1095-C, you may request one by contacting one of the following Human Resources Generalist, Rilee Conrad or General Manager, Corbin Linder as follows:

Your request must include first and last name, address, your contact number, and e-mail address (if you want an electronic copy of the Form 1095-C). You should expect to receive your Form 1095-C within thirty (30) days of your request.
